We are currently looking to fill a CCBHC Triage Specialist role at our Merrionette Park office.
SALARY: The average starting salary for this position will fall in the range of $59,000 and $70,000 annually. Where candidates fall in this range will be based on skill and experience level. Where candidates fall in this range will be based on skill and experience level. Of note, is that candidates possessing a LCSW or LCPC may be considered for a possible salary up to $78,000.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Clinical Triage & Assessment: Conduct initial screenings and comprehensive diagnostic assessments (mental health and substance use) to determine the appropriate level of care.
IM+CANS Completion: Use the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM+CANS) to document clinical needs, risk factors, and strengths for all new clients.
Crisis Intervention: Provide immediate de-escalation, safety planning, and short-term crisis counseling for individuals in acute distress.
Treatment Planning: Facilitate the initial Person-Centered Treatment Plan in collaboration with the client and their family, ensuring alignment with CCBHC quality measures.
Care Coordination: Work with Peer Support Specialists, Case Managers, and Clinicians to provide "warm handoffs" to internal programs.
Documentation: Maintain timely and accurate electronic health records (EHR) in compliance with 42 CFR Part 2 and HIPAA standards.
Effectively work with diverse staff and service population.
Other duties as assigned.
KNOWLEDGE, SKILLS, and ABILITIES:
Good computer skills and ability and willingness to learn a variety of applications. Proficiency with Microsoft Office programs.
Must be detailed oriented with good organizational skills.
Excellent communication skills (verbal and written).
Excellent interpersonal skills.
Ability to multitask within a fast-paced environment and meet multiple deadlines simultaneously.
Demonstrated courtesy, patience, and good judgment under pressure.
Ability to work effectively with diverse individuals and groups.
QUALIFICATIONS:
Master’s degree in social work or a related behavioral science field. from an accredited college or university required.
Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC); IL Qualified Mental Health Provider (QMHP) Required
Minimum 2 years of experience in behavioral health, crisis intervention, or intake settings.
Must obtain IM+CANS Certification within 30 days of hire
Proficiency in using HFS electronic eligibility systems (MEDI/REV) and Electronic Health Records.
Bilingual (English/Spanish) preferred.
Knowledge of ASAM criteria for substance use disorder triage preferred.
Experience serving Veterans and their families preferred.
Ability to complete required 40-hour domestic violence training, preferred.

PHYSICAL DEMANDS
While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle, and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch. The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing, and viewing a computer terminal.
