We are seeking a dedicated and compassionate Case Manager to join our BH Palos Hills and Merrionette Park team.
In this role, you will provide comprehensive services tailored to a diverse client population, helping enhance their potential and promote optimal functioning within the community. Working in collaboration with a supervisor, you will address emergency and crisis situations within the target population. This position operates across various settings, including office environments, client homes, and community-based locations, ensuring culturally competent service delivery for individuals from diverse backgrounds.
The Case Manager’s key responsibilities include promoting the importance of care coordination to improve clients’ physical, emotional, and behavioral health outcomes. Under general supervision, the Case Manager will utilize intermediate-level expertise to conduct in-depth client assessments, manage more complex cases, and develop customized service plans. Working closely with clients, you will create actionable plans with clear goals and resources, continuously reviewing and updating these plans to meet evolving needs and deliver impactful support.
SALARY: The average starting salary for this position will fall in the range of $40,000 to $46,000 annually. Where candidates fall in this range will be based on skill and experience level.
$2,500 sign on bonus ($500 on the Second Paycheck; $1000 at 6 months; and final payment of $1000 on the 12 month anniversary).
ESSENTIAL JOB FUNCTIONS:
Conducts thorough assessment of clients. Evaluates problem and service needs.
Develops a written client service plan specifying goals to be achieved and services to be pursued.
Supplements service plans to meet client needs.
Provides, as appropriate, supportive help, advocacy and case management to individuals and families in coping with environment and interpersonal relationship
issues.
Works collaboratively with community resources. Develops a knowledge of resources and helps clients to utilize them.
Works within contract, grant, and program requirements including meeting timelines and reporting requirements.
Engages client in settings that may present a challenge for the client for the purpose of learning/practicing/developing healthy behaviors and coping skills.
Reassesses and monitors client situation and adequacy of service plan.
Collaborates and participates with other program staff in the planning and implementation of service plans.
Subsequent to consultation with supervisor, handles emergency and crisis situations within target population.
With supervisory approval, authorizes limited amounts of financial assistance to own clients.
May work with community groups.
Works effectively with diverse staff and service population.
Other duties as assigned.
QUALIFICATIONS:
Bachelor's Degree in Psychology, Social Work, or related field from an accredited college or university.
One year of case management experience in a social service or healthcare setting preferred.
Certified Alcohol and Drug Counselor certification preferred.
Bilingual in Spanish preferred.
Crisis stabilization experience preferred.
Ability to work evening and weekend hours as needed.
ADDITIONAL REQUIREMENTS:
Satisfactorily pass all relevant screening and criminal background checks.
TRAVEL/TRANSPORTATION REQUIREMENTS:
Driving for work required with transportation of clients preferred in a personally owned vehicle.
PHYSICAL DEMANDS
While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle, and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch. The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing, and viewing a computer terminal. Must be able to lift at least 30 lbs.
